This form serves two purposes:
(1) Alumni can use the top portion of the form to provide contact information changes.
(i.e. name, mailing address,
email change, etc.) Just complete the necessary
information below and click "Submit Form" at bottom of page.
(2) Alumni can provide an electronic Chieftain update by following one of the
following two procedures:
Preferred method:
Download the questionnaire in
Microsoft Word format by
clicking here. (It takes a few seconds to open Word.)
Save the file on your PC and complete the questionnaire at your convenience.
When you've completed the Word document, send an e-mail to:

attaching the Word document.
